UK MoD receives Crown Censure from HSE for 2016 soldier's death during unsafe training exercise.
The UK Ministry of Defence (MoD) has been issued a Crown Censure by the Health and Safety Executive (HSE) following the accidental death of a soldier during a training exercise in 2016. Conor McPherson, 24, was fatally shot in the head by a fellow soldier at the Heely Dod firing range in Otterburn, Northumberland. The HSE found the MoD failed to properly implement a safe system of work for the exercise, resulting in a poor planning and conduct of the training mission.
