Staff Sgt John McKelvie died from a brain injury during a training exercise due to inadequate supervision and safety enforcement, with the MoD aware of vehicle deficiencies.

Family claims the MoD failed Staff Sgt John McKelvie who died from a brain injury after his armoured Supacat Jackal rolled down a hill during a training exercise at Catterick Garrison. A five-day jury-led inquest found that lack of supervision by the chain of command and inadequate safety enforcement contributed to McKelvie's death. The Ministry of Defence was aware of credible deficiencies in the vehicle, with issues discovered nine years before the crash.
