27 train cars derailed near Silton, Saskatchewan in October 2021 due to a broken rail with pre-existing fatigue cracks. The Transportation Safety Board report found that although the track was tested more frequently than mandated, the fatigue cracks remained undetected, leading to rail breakage when the train passed over it. Canadian Pacific Railway has since implemented a detector system to prevent similar incidents.
