Shannon Valley Plant Hire Ltd and Behan Aggregates & Recycling Limited were fined a combined €650,000 following the death of worker Christopher Miley at Behan Quarry. Miley was knocked down by a digger while refuelling vehicles in dark and wet conditions, with no safe system in place for refuelling outside daylight hours, no traffic marshal, and no designated lit area. Both companies pleaded guilty to failing to ensure health and safety measures for their workers.
