Openreach fined £1.34m for engineer's death in North Wales, lacking water safety protocols.

Telecoms firm Openreach Limited was fined £1.34 million after an engineer, Alun Owen, died in a swollen river while attempting to repair a telephone line in Abergwyngregyn, North Wales. The Health and Safety Executive (HSE) found that Openreach had no safe system of work in place for staff working near water and that Owen and others had not received necessary training. The company admitted to breaching Section 2 (1) of the Health and Safety at Work etc. Act 1974.
