A New Zealand mother's baby died after critical scan results indicating low amniotic fluid and fetal weight were not communicated to her midwife due to a "coding error" at Pacific Radiology. The Health and Disability Commissioner found both Pacific Radiology and the midwife breached the woman's right to care. Pacific Radiology was ordered to apologize and implement new safeguards to ensure urgent cases are prioritized.
