A woman with liver disease at high risk for cancer passed away after a significant delay in her diagnosis. Regular check-ups stopped in 2019, and she was only diagnosed in 2022 after experiencing nausea and upper back pain. Health New Zealand breached the Code of Health and Disability Services Consumers' Rights for not scheduling follow-up ultrasound scans and not booking an outpatient appointment with a gastroenterologist.
