The Health and Disability Commissioner found a GP breached the Code of Health and Disability Services Consumers' Rights for not following up with a patient who had been referred for an ultrasound. The woman, in her 60s, attended a consultation for post-menstrual bleeding but did not undergo the ultrasound as she believed it was optional. A year later, she was diagnosed with stage 4 endometrial cancer.
