Health New Zealand | Te Whatu Ora Waitaha Canterbury (formerly Canterbury DHB) breached the Code of Health and Disability Services Consumers' Rights by failing to detect a man's renal cancer three times. The man had been under vascular surgery services at Christchurch Hospital since 2010. The Deputy Health and Disability Commissioner found the health authorities missed three opportunities to spot the disease.
