A coroner's inquest found that Queensland mother Charmaine McLeod, who killed herself and her four young children in a fiery car crash in 2019, acted "intentionally" and not "protectively." Charmaine McLeod's actions were deemed "deliberate," and it was virtually impossible to predict or prevent the tragic incident. McLeod had a complex mental health history, including untreated borderline personality disorder and past suicide attempts. Following the crash, the coroner recommended better training, support, and guidelines for legal practitioners handling cases involving clients with domestic and family violence and complex mental health issues.
