A deckhand drowned off Wick, Scotland, while working on the fishing vessel Kingfisher during a creel shooting activity. He became entangled in a rope, pulling him overboard despite his personal flotation device (PFD) inflating. Rescue efforts failed. The Marine Accident Investigation Branch (MAIB) identified the PFD's safety loop as a snagging hazard and recommended improved risk assessments and hazard mitigation for the fishing industry.
