21-year-old Auckland law student Farzana Yaqubi was murdered eight weeks after reporting stalking allegations against a man. The Independent Police Conduct Authority found the police's initial assessment matrix was "not fit-for-purpose" as it did not consider all lines of enquiry and the risk posed to victims. The police also failed to link Yaqubi's complaint with another complaint from a young girl alleging threats from the same man. Police have since acknowledged the IPCA's findings and accepted their response to Yaqubi's reports was inadequate, missing multiple opportunities for earlier intervention.
