East London coroner Nadia Persaud has warned that the Metropolitan Police and Probation Service face potential future fatalities due to systemic failures, after the murder of law graduate Zara Aleena. Persaud's report highlighted concerns including low staffing levels, poor risk assessment understanding, and a lack of professional curiosity within the Probation Service, as well as a lack of "rigour, detail, and independence" in the Met's investigation of the case. These vulnerabilities could have fatal consequences, according to Persaud, who has sent copies of her report to key figures such as the Chief Probation Officer, Justice Secretary, Commissioner of the Met, Head of Redbridge Council, and Home Secretary, demanding responses by September 20th with plans to improve.
