Gary Chapman, a 33-year-old coal miner from South Williamson, Kentucky, died in an accident at the Mountaineer II Mine in Logan County, West Virginia. His death is the fourth mining fatality in the state this year. Investigations are being conducted by the West Virginia Office of Miner's Health, Safety, and Training, and the Mine Safety and Health Administration.
