1-year-old Philadelphia I-95 bridge crash caused by unsecured gasoline tanker hatch.

In Philadelphia, a year after a deadly crash destroyed a bridge along I-95, federal investigators have revealed that gasoline spilled out from an unsecured hatch on top of the gasoline delivery tanker that overturned. Video evidence showed that the cover of a 16-inch manhole was open when driver Nathaniel Moody arrived to refill the tank and was not properly secured when he drove away. The crash killed Moody and disrupted transportation for weeks.
