A toddler died after suffocating at a nursery due to staff negligence.

A 14-month-old boy died after suffocating at a Dudley nursery in December 2022. Staff restrained the child face down on a cushion with a blanket over his head. The nursery has since admitted corporate manslaughter and safety charges, while staff and the owner also pleaded guilty to manslaughter and health and safety offenses.
