Gauteng's premier took blame for a 2024 mental health patient’s death due to systemic hospital failures.
Gauteng Premier Panyaza Lesufi accepted responsibility for the 2024 death of mental health patient Lerato Mohlamme at Dr George Mukhari Hospital, following a Health Ombud report exposing systemic failures including illegal admission, withheld medication and food, inadequate staffing, and poor infrastructure.
The report revealed punitive practices, falsified records, and unreported abuse allegations.
In response, the province has hired 12 nurses, added nine security staff, installed CCTV, completed fire safety upgrades, and trained staff.
Professional bodies will review misconduct claims.
The government also addressed a separate neonatal death linked to medication errors and weak ICU oversight.
