A 12-year-old non-swimmer died in 2019 during a river boarding trip on the River Tay after getting trapped behind a weir, despite safety warnings and a prior company fine for a 2024 safety breach.
A 12-year-old boy who could not swim was allowed to attend a 2019 river boarding event on the River Tay, leading to his death after he became trapped behind a weir, according to a Fatal Accident Inquiry.
The Church House Community Group, which organized the outing, said non-swimmers could participate if comfortable in water, relying on provider Outdoor Pursuits Scotland Ltd to ensure safety.
In 2024, the company admitted a health and safety breach and was fined £10,000.
Warning signs about strong currents and deep water were present but not noticed.
The inquiry also examines the 2024 drowning of a 39-year-old man attempting a rescue.
The FAI is expected to continue for over 10 days.
