An 84-year-old man, Terrence Frost, died at Ipswich Hospital in July 2024 after a delayed diagnosis of sepsis, despite multiple hospital visits and a GP’s failed attempts to contact hospital staff for over 30 minutes.
He waited five hours in emergency care without a diagnosis before suffering a cardiac arrest.
Suffolk’s senior coroner issued a prevention of future deaths report, warning that communication failures at the hospital could endanger patients, especially in sepsis cases.
The East Suffolk and North Essex NHS Trust has until May 4 to respond.
