A 99-year-old dementia patient, Leonard Hewgill, died in October 2018 after being assaulted by another resident, Ike Cowley, at the Hilda Ross Special Care Unit in Hamilton, New Zealand.
The attack, which occurred on October 3, caused a head injury, and Hewgill died three days later.
Cowley, also with dementia, had a history of 54 documented aggressive incidents but was not restrained.
Staff failed to implement proper de-escalation or risk management, and the facility lacked adequate training and documentation.
Cowley was charged but deemed unfit to stand trial due to his condition and released.
