The republican group Lasair Dhearg claimed responsibility on social media, citing Queen Victoria’s role during the Irish potato famine and calling for removal of imperial symbols in a future socialist republic.
The Belfast Health and Social Care Trust said staff were working to remove the paint.
Unionist politicians, including DUP’s Phillip Brett and Ulster Unionist’s Alan Chambers, condemned the act as an attack on heritage, public space, and the healthcare system, stressing that damage to a hospital diverts funds from essential services.
