Australia's new home-based end-of-life program offers up to $25,000 for dying patients to stay home, with care access guided by medical assessment and provider choice.
A new end-of-life care program in Shellharbour, Australia, launched in November 2025, provides up to $25,000 over 12 weeks for older adults with less than three months to live who wish to stay at home.
Part of the Support at Home initiative, which replaced prior home care programs, the funding aims to improve care access and community connection, following Royal Commission recommendations.
Eligibility requires a doctor or nurse practitioner’s assessment, after which individuals choose their provider.
While some face barriers due to provider participation gaps, local palliative care provider Sisternightingale Home Nursing offers support, including 24-hour clinical care and guidance for all, regardless of service use.
The Commonwealth Home Support Program is expected to transition to Support at Home by no earlier than July 2027.
