A Tipperary couple, Sinead O'Donnell and Luke Kelly, received a formal apology from Tipperary University Hospital in January 2026 for failures in care during the birth of their son Freddie, who died stillborn on July 2, 2021.
The hospital, acting through the HSE, admitted breaches in duty, including inadequate fetal monitoring, delays in responding to urgent signs, and slow medical intervention.
Despite a normal fetal heart rate seven minutes before delivery, Freddie was born deceased after a Caesarean section.
A post-mortem found no structural abnormalities.
The apology acknowledged the emotional toll on the family and admitted the care fell short, with Mr. Justice Paul Coffey expressing sympathy for their loss.
