The De Havilland DHC-2 Beaver lost control during takeoff from St-Mathias aerodrome, struck the water, and overturned, killing the pilot while the passenger escaped.
The TSB found a critical control chain was improperly reinstalled during a February 2025 maintenance check.
The repair company has since added an independent verification step.
The board urged thorough pre-flight checks after maintenance and stressed the need for all seaplane occupants to wear personal flotation devices.
