South Yorkshire Police are investigating a serious error that led to the family of 17-year-old Trevor Wynn being wrongly told he died in a December 13 crash in Rotherham, when he was actually hospitalized with serious injuries.
Forensic testing later revealed that 18-year-old Joshua Johnson had died, correcting the misidentification.
The force has referred itself to the Independent Office for Police Conduct to review its identification procedures.
