A Maryland DHS investigation found Fenwick Behavioral Services staff negligent in the September 2025 suicide of 16-year-old foster girl Kanaiyah Ward, who died from a Benadryl overdose in a Baltimore hotel room.
The report revealed a worker was assigned a 53-hour shift without rest, failed hourly checks, and medications were not secured.
Three staff members were cited for approving the extended shift and failing to lock up drugs.
The findings were sent to the Baltimore state’s attorney, who may pursue charges.
DHS has ended hotel placements for foster youth and stopped using Fenwick for one-on-one care.
