A Southern Railway train collided with buffers at London Bridge station on December 13, 2024, after its driver fell asleep due to fatigue, the Rail Accident Investigation Branch (RAIB) reported.
The driver, who had worked nine of his scheduled 12 rest days in 22 days and was awakened early due to childcare changes, suffered a microsleep.
The train entered the platform at 13.3 mph, slowed to 2.3 mph at impact, and struck the buffers at 3:45 p.m.
The emergency brake was applied too late, but no injuries occurred and damage was minor.
Safety systems did not activate because the speed was below their intervention threshold.
The RAIB recommended improved fatigue management, prompting Govia Thameslink Railway to update its fatigue risk policies, including scientific shift planning and staff engagement.
