A September 18, 2025, Optus network outage caused by errors during a routine upgrade disrupted triple-zero emergency calls across large parts of Australia for nearly 14 hours, preventing 455 of 605 emergency calls from connecting and contributing to two deaths.
An independent review found at least 10 mistakes by Optus and its contractor Nokia, including flawed instructions, ignored early warnings, and systemic gaps in accountability, escalation, and company culture.
It took about 13 hours to fully recognize the issue.
The report, led by Kerry Schott, recommended 21 improvements, including better change management and breaking down siloed operations.
Optus has committed to implementing all changes, with disciplinary actions, including possible firings and financial penalties, underway.
Chairman John Arthur apologized, calling the outage unacceptable.
