England’s NHS launches nationwide system to real-time flag maternity safety risks, aiming to prevent harm.

The NHS has launched the Maternity Outcomes Signal System (MOSS), a real-time monitoring tool rolled out across all 71 maternity units in England to detect safety risks in maternal and newborn care. Using statistical analysis of routine data, MOSS issues amber (95% confidence) or red (99% confidence) alerts when outcomes deviate significantly from national averages, triggering urgent reviews and corrective actions. The system, developed after past maternity service failures, aims to prevent avoidable harm by enabling early intervention. It has already been tested at pilot sites and is now operational nationwide, with findings to be reported to regional and national teams and discussed at public board meetings.
