Oxford County Paramedic Services has won an Ontario Health System Quality & Innovation Award for its Remote Patient Care Monitoring and Chronic Disease Management Program, which trains paramedics to conduct in-home health checks and remotely monitor patients with conditions like COPD, heart failure, diabetes, and hypertension.
Since its launch in August 2022, the program has reduced 911 calls by 61% and emergency room visits by 72% within six months, with 72% of participants reporting less need for emergency care.
It achieved a 95% patient satisfaction rate in 2024-25 among 224 participants, and support from local health and community partners helped deliver personalized, proactive care that improves health outcomes and reduces strain on the healthcare system.
