CMS launches 10-year model in 2026 to reward Medicare providers for better chronic disease outcomes using tech.
The Centers for Medicare & Medicaid Services (CMS) launched the 10-year ACCESS Model in 2025, beginning July 2026, to improve chronic disease care by rewarding Medicare Part B providers for continuous, tech-enabled care that achieves better outcomes in conditions like diabetes, hypertension, and mental health.
Providers will be paid based on measurable results such as improved blood pressure control and reduced hospitalizations.
Electronic Caregiver, Inc., a New Mexico-based health tech company, offers its Addison Care platform to help providers meet model requirements through daily patient engagement, remote monitoring, behavioral health support, and EHR integration.
While Electronic Caregiver is not a direct participant and does not guarantee eligibility, it is expanding services and developing tools to support organizations applying for the model, with CMS expected to release approved participants in late 2026.
