A coroner has ruled the death of 16-year-old Niamh McNally at University Hospital Limerick on January 29, 2024, was a medical misadventure due to multiple missed opportunities in her care.
Despite a known congenital heart condition and symptoms of severe breathing difficulties and coughing up blood, timely cardiology evaluation was delayed by nine days, and diagnostic tests were deemed inadequate.
She died from asphyxiation caused by a pulmonary hemorrhage likely from a ruptured collateral artery.
Her family said concerns were ignored, and her final words were “Mammy, I can’t breathe.” The inquest revealed systemic failures, with the HSE issuing an apology and 15 recommendations for change.
This marks the third similar case at UHL in under a year, raising concerns over patient safety and hospital accountability.
