Despite these reports, no action was taken.
Officers responded to a welfare check for Leon Woods, a registered sex offender with prior child molestation convictions, after his daughter feared for his safety.
They discovered the children during the visit.
Woods and Carol Phillips now face aggravated child neglect charges, with Woods also charged for violating sex offender registration laws.
DCS has not responded to inquiries about its handling of prior reports.
