A family sues an Ohio nursing home over the death of their 93-year-old father, who fell fatally despite being a high fall risk and lacking required supervision.
A family is suing the Avenue at Aurora Care and Rehabilitation Center in Ohio after their 93-year-old father, Roger Andrews, died from a fall in January 2025.
The lawsuit, filed November 12, alleges the facility failed to provide required one-on-one supervision despite Andrews being assessed as a high fall risk, leading to three falls in five days, including a fatal face-first fall from his wheelchair.
The medical examiner ruled the death an accident due to blunt impact from falls, but the family claims negligence and wrongful death, naming both the nursing home and its parent company, Progressive Quality Care.
The facility and company have not responded to requests for comment.
