The data, including mental health diagnoses and test scores from Chapter 980 evaluations through October 2022, was sent to a Kenosha state agency on July 17 and violated HIPAA.
Discovered September 16, the breach prompted the DOC to demand deletion of the files, notify affected individuals, and report the incident to federal health authorities.
