A 67-year-old man from Broken Hill, Dennis Charles Jackson, died in January 2019 from pneumonia after undergoing unnecessary lung surgery due to a lab error that falsely diagnosed him with lung cancer.
In July 2018, his biopsy was contaminated with another patient’s cancerous tissue, likely from inadequate equipment cleaning during a late-night shift.
Despite no cancer being found during surgery, the incorrect diagnosis led to major complications.
South Australian Deputy State Coroner Ian White concluded the contamination contributed to Jackson’s death and recommended an independent review of SA Pathology’s safety protocols.
