A foster care service in southeast Ireland failed national standards for the second year, with children lacking proper social workers and oversight.
A foster care service in Carlow, Kilkenny, and South Tipperary, Ireland, failed to meet three of five national standards for the second straight year, according to a July 2025 HIQA inspection.
The review found many children lacked assigned social workers, with some relying on unqualified staff or no worker at all, and oversight systems were deemed insufficient.
While Tusla reported improvements, including assigning every child a social worker and implementing new audit processes, HIQA noted ongoing issues with care plan updates, review follow-through, and safeguarding.
The agency acknowledged staffing shortages but said it prioritizes high-risk cases.
