The inquest found systemic failures, including understaffing, overreliance on temporary staff, inadequate risk assessments for blood clots and mobility, poor wound care, and a transfer from a Doncaster facility in April 2024 without clinical justification.
The jury determined these factors likely played a significant role in her death, which followed self-harm on May 12 and inadequate supervision.
Assistant coroner Alexandra Pountney will issue a Prevention of Future Deaths Report to the involved NHS trusts.
