Optus executives depart in 2026 after outage disrupted 600+ emergency calls, linked to three deaths.
Optus executives Michael Venter and Mark Potter will leave the company in March 2026 following a September outage that disrupted over 600 emergency triple-zero calls across multiple Australian states, linked to three deaths.
The failure, caused by a faulty firewall update, was worsened by Optus sending alerts to the wrong email, delaying federal response by more than a day.
The incident drew national criticism, with Prime Minister Anthony Albanese stating Optus had "let down the nation," and Singapore’s Prime Minister Lawrence Wong offering condolences.
CEO Stephen Rue said the departures were part of long-standing discussions and emphasized continuity, with Andy Giles Knopp of Aussie Broadband set to succeed Venter in April 2026 and John McInerney of OneNZ taking over as CIO.
Optus and Telstra also warned customers that older Samsung phones may lose network access due to triple-zero connectivity issues.
