Global health organizations have released updated guidelines to combat postpartum hemorrhage (PPH), a leading cause of maternal death, with nearly 45,000 deaths annually.
The new recommendations, based on the largest study to date, lower the threshold for diagnosing PPH to 300 mL of blood loss or abnormal vital signs, urging earlier intervention.
They promote the "MOTIVE bundle"—measuring blood loss, administering uterotonics, using tranexamic acid, and escalating care when needed—and emphasize prevention through antenatal iron supplementation and avoiding routine episiotomies.
The guidelines support misoprostol use in low-resource areas and call for stronger midwifery and health systems, part of the Global Roadmap for PPH 2023–2030, launched at the 2025 FIGO World Congress in Cape Town.
