An investigation revealed that police officers and ambulance staff in Northern Ireland failed to properly care for a 19-year-old, Shannon McQuillan, and her boyfriend, leaving them alone on the roadside, which led to her fatal accident.
The Police Ombudsman reported "wholly inadequate" communication and multiple breaches in duty of care, such as failing to complete basic checks and recognize their vulnerability.
The PSNI accepted the findings and is reviewing its policies.
The McQuillan family has filed a lawsuit, believing Shannon's death could have been prevented.
