An Irish electrical engineering company, John Fletcher Ltd, was fined €400,000 for the 2017 workplace death of employee Alan Mulhall, a father of four, who was fatally injured when a 760kg switchgear fell on him at St Patrick's Hospital.
The company and a director admitted to failing to discharge their duties as employers, as required by the Safety, Health and Welfare at Work Act 2005.
The Health and Safety Authority criticized the risk involved in manually moving the heavy equipment.
