Human errors led to a fatal collision between a Japan Airlines plane and a Japan Coast Guard aircraft at Haneda Airport.

A report from the Japan Transport Safety Board on the January collision between a Japan Airlines (JAL) plane and a Japan Coast Guard (JCG) aircraft at Haneda Airport has identified human errors as the main cause. The JCG crew mistakenly believed they had clearance to enter the runway, while the air traffic controller failed to notice their presence. The JAL pilots did not spot the JCG aircraft before landing, causing a collision that killed five Coast Guard crew members but allowed all 379 JAL passengers to evacuate safely.
