A New Zealand supervising pharmacist was found to have violated a patient's rights by mistakenly providing Parkinson's disease medication intended for someone else. The error occurred when a faxed prescription, including pages for different medications, wasn't properly verified. The patient experienced dizziness and imbalance. The pharmacy has since retrained staff, updated procedures, and switched to ePrescriptions to prevent future errors.
