Malcolm Houghton, a 68-year-old veteran HGV driver, died after his vehicle crashed into a stationary HGV on the A590 near Kendal, Cumbria, on October 12. The investigation report suggests he likely suffered a medical episode, causing him to lose control of his vehicle. There were no signs of braking before impact, and the coroner concluded his death was instantaneous due to head and neck injuries.
