The Health and Disability Commissioner found two psychiatric social workers at fault after a man with schizophrenia took his own life following their inadequate assessment. They failed to consider his suicidal ideation, relied on self-reporting, and did not consult other professionals. Although criticized for their actions, both social workers have undergone further training, and Health New Zealand has revised its training policies in response to the incident.
