2021 Kentucky hospital nearly harvested organs from alive patient; investigation ongoing.

In October 2021, a grave mistake nearly led to organ harvesting from a living man at Baptist Health hospital in Richmond, Kentucky. Natasha Miller, an organ preservationist, recognized that Anthony Thomas "TJ" Hoover II was alive during the procedure, prompting two doctors to refuse participation. The operation was canceled, and several KODA employees later quit, claiming the incident was downplayed. Investigations by the Kentucky attorney general and federal health authorities are ongoing.
