2022 Cache Creek collision caused by CPR supervisor's error in track permit cancellation.

A Transportation Safety Board report identified a Canadian Pacific Railway (CPR) supervisor's error as the cause of a December 29, 2022, collision in Cache Creek, B.C., between a freight train and a work vehicle. The supervisor mistakenly canceled the wrong track permit, leaving the area unprotected, resulting in the crash. Fortunately, there were no injuries. CPR has since enhanced its safety procedures regarding track occupancy permits in response to the advisory.
