A report by Deputy Commissioner Rose Wall found that Health New Zealand Te Tai Tokerau violated a woman's rights by failing to provide adequate care for her Type 2 diabetes, resulting in multiple hospital admissions and her eventual death from a bacterial infection. The hospital did not properly assess her ulcers or refer her to necessary services. The report also criticized the community podiatry service for its inadequate referral processes. Health NZ has since made improvements.
