A report by former Abbotsford Police chief Bob Rich found that Blair Donnelly, accused of a triple stabbing at a Vancouver festival, had been granted 99 unescorted leaves from the BC Forensic Psychiatric Hospital without incident prior to the attack. The report, commissioned by the B.C. government, did not find policy breaches but recommended improvements in managing high-risk patients. The B.C. Ministry of Health has accepted and started implementing these recommendations.
